Improving UEC services for older people living with frailty

This webinar will explore how CLEAR supported Frimley Health NHS Foundation Trust in Surrey to improve UEC services for older people living with frailty.

Sponsored by NHS Frimley Integrated Care Board, the project examined pathways for patients attending the emergency departments at Frimley Park and Wexham Park Hospitals and explored ways to reduce demand on UEC services, avoid unnecessary hospital admissions and improve patient experience. Recommendations for changes included unifying frailty care, extending the working hours of the frailty service, enhancing the trust’s IT system to improve the coding and efficiency, introduction of SDECs and more collaboration with community teams to streamline referral and discharge processes. CLEAR is now supporting the trust to implement the recommendations.

Clinicians will discuss their experiences of driving the project, the challenges they faced, processes around designing the new models of care and implementation of the proposals.
