The CLEAR project identified the following issues:

  • Significant congestion in A&E from limited outflow of patients requiring inpatient admission.
  • Ineffective flow through rapid assessment and treatment.
  • Challenges associated with managing specific patients, such as people with mental health needs and frailty.

New ways of working including:

  • Senior decision-maker early in the patient journey.
  • Agile and responsive triage and treat.

New models of care:

  • The introduction of an assessment zone for low acuity, non-emergency patients.
  • Streamlined pathways for specific patient cohorts.
  • Better care for patients and decongestion in A&E by redirecting people to more appropriate care settings.
  • Better outcomes for patients as a result of senior decisions early on.
  • Improved efficiency and patient flow through A&E.
  • Reduced emergency admissions and lengths of stay.
  • Potential savings identified as £2.6 million per annum.
