We’re thrilled to share that a dementia case management service developed through our CLEAR Proactive Care Programme has been shortlisted for the 2025 Nursing Times Awards, and we couldn’t be prouder of the incredible team at Healthy Hyde Primary Care Network (PCN).
This nurse-led service, designed and led by Emma Maguire, Dementia Case Management Service Lead, at Hyde PCN, has delivered exceptional results: a 27% reduction in emergency hospital admissions in its first year and over £1 million in hospital cost savings within two years. On top of that, it delivered £160,000 in local savings, enough to fund eight extra GP sessions per week. These are the kinds of transformative, high-impact outcomes the CLEAR programme is designed to deliver.
The project is shortlisted in the Nursing in Primary Care category, which recognises innovative, nurse-led initiatives that improve patient care, strengthen services, and offer models that can be scaled across primary care.
Emma and the Hyde team addressed a significant area of need: supporting people with dementia living at home to reduce emergency admissions. They developed a truly proactive, personalised service. Their model uses nurse case managers and navigators to coordinate care, provide a single point of contact for families, and strengthen links across community support. It’s a perfect example of what the CLEAR programme is all about: clinically led, data-driven, impactful care redesign.
