The National CLEAR programme is delighted to announce that Central and West Warrington PCN has been shortlisted for the data-driven transformation award at the HSJ Awards for its work on the CLEAR Living Well project to improve proactive identification and care of cardiovascular disease (CVD), recognising an outstanding contribution to healthcare and securing a place at the prestigious awards ceremony later this year.

Central and West Warrington PCN data showed that their population is at increased risk of CVD, with evidence of health inequalities, a low uptake of NHS health checks and lack of referrals to local lifestyle management. The PCN was sponsored by NHS England to participate in a 26-week national initiative for CVD prevention in primary care. The PCN wanted to ensure they were as effective as possible at delivering traditional interventions for CVD, whilst also being innovative and considering new ways of working and using their workforce to provide good and sustainable anticipatory care. They wanted to increase and improve proactive identification and care of cardiovascular disease (CVD) for their population and target at-risk groups. They were also looking to streamline NHS Health Checks and expand the service by offering an enhanced health check, regardless of age, to those at greatest risk of CVD.

PCN staff participating in the CLEAR Living Well implementation project developed and implemented two new services to improve and widen screening for CVD risk factors, improve quality and consistency of proactive care services and prevention of adverse health outcomes.

The ‘Reduce your risk’ project at Central and West Warrington PCN has increased engagement and access for hard-to-reach communities, while diverting appointments away from general practice. As well as improving patient care, the PCN has increased the offer for NHS Health Checks at minimal additional cost or estate demand, and successfully removed workload from GPs and nurses. The project has empowered the workforce to work differently, improving transformation capability and confidence. The PCN is on track to exceed its target of seeing 75% of patients eligible for an NHS Health Check.
