Supporting primary care networks to develop new care pathways for the prevention of CVD

This webinar will give an overview of our CLEAR Living Well CVD prevention toolkit and discuss how it can be used to develop better CVD prevention.

Commissioned by NHS England, the CLEAR Living Well programme has been working with sites across the country to design, implement and spread new models of care for the prevention of CVD – which remains one of the biggest causes of avoidable death.

As part of the programme, a practical toolkit has been developed to support PCNs, ICSs and other primary care partners to improve existing CVD prevention pathways or embed new ones into their service – with the aim of providing more targeted, personalised and proactive care for CVD prevention.

The toolkit offers a mix of transformation methodology and practical support encompassing seven different models of care, which were developed and tested by pilot PCN sites as part of the programme.

The webinar will include an overview of the toolkit, details of current and future national CVD prevention priorities from NHS England colleagues and next steps for the CLEAR Living Well programme.
