Transforming mental health services enhancing pathways and improving access

This session examined how CLEAR supported Cornwall Partnership, Cheshire and Wirral Partnership and Norfolk and Suffolk NHS Foundation Trusts to design new models of care to transform key mental health services.

The project at Norfolk and Suffolk NHS Foundation Trust looked at improving care of older adults with mental health needs including enhancing care pathways for patients with dementia to avoid unnecessary hospital admissions and prolonged lengths of stay.

Cheshire and Wirral Partnership NHS Foundation Trust’s project explored ways to improve care for young people transitioning from child to adult mental health services including reducing demand on crises services.

There were two CLEAR projects at Cornwall Partnership NHS Foundation Trust – project one looked at improving care for adults requiring mental health support that didn’t meet criteria for both community and secondary care service; project two focused on aligning community mental health teams with PCNs to improve integration of services and patient access.
