CLEAR Anticipatory Care has officially started, aimed to support developments of anticipatory care across Primary Care Networks (PCNs) and Integrated Care Systems (ICSs).
The National CLEAR Programme is working with six PCNs across the country to support them in developing Anticipatory Care approaches using unique data insights and to innovate novel solutions. The six PCNs involved are: Hyde, TABA, East Warrington, East Lindsay, Camden and Regis. Clinicians from each PCN will be trained in the CLEAR methodology where they will learn the fundamentals of clinicals engagement and data analytics, allowing them to redesign new models of care and workforce. These innovations will support PCNs to deliver proactive personalised care in the community to patients with complex needs. This work will be contributing to a shared learning exercise of anticipatory care across England, in partnership with NHS England and NHS Improvement.

Anticipatory Care is designed to support those patients who are at high risk of unwarranted health outcomes to live well and independently for longer, through structured proactive care. This supports the prevention of ill health in line with the NHS Long Term Plan, with the ambition that people will be able to enjoy at least five extra healthy, independent years of life by 2035.

“I am very excited to be supporting our Anticipatory Care projects. Being a former CLEAR Associate myself, I’m looking forward to working closely with our six sites and developing the new Associates through the Programme. Exciting times ahead!”
