Primary care networks in England are invited to put forward expressions of interest (EOI) for CLEAR transformation support to develop new models of care for the prevention of cardiovascular disease (CVD).

Commissioned by NHS England, 14 PCNs – two from each NHS England region – are being offered the opportunity to take part in a five-month project to develop new models of care for people at risk of developing CVD to help them to lead longer, healthier lives.

The support is part of ongoing nationwide work focused on key NHS Long Term Plan ambitions and the national CVD programme. It will be based on preventative new models of care and workforce that were developed with frontline staff at three PCNs in Cheshire and Merseyside as part of CLEAR Living Well 2023.

The programme will comprise three regional workshops and a series of one-to-one meetings when PCNs will be assisted to build a business case for the new model of care. Through the workshops, the PCNs will be guided through the CLEAR approach including clinical engagement and data analysis to identify patients at risk of developing CVD and other related long-term conditions.

The 14 PCNs will be selected through an EOI process with the programme split into two phases, phase one with six PCNs from January to May 2024, followed by phase two from May to September 2024. Two webinars are being held when the National CLEAR team will explain the project in more detail and answer questions. The dates are: Tuesday 12 December (13:00 – 14:00) and Wednesday 13 December (12:00 – 13:00). Please follow the links to join.

The deadline to send an EOI is 5pm on 18 December 2023. For further information on the opportunity, please read our EoI information pack. To submit an EOI, please complete our EOI form.

Confirmation of the 14 successful PCNs will be made on Thursday 21 December 2023.
